What You Can Do

  • Tell your care team about the things that matter to you.
  • Make a list of questions to ask at each health care visit.
  • Talk with your family and providers about your wishes for care at the end of life.

If medication is necessary, use Age-Friendly medication that does not interfere with What Matters to the older adult, Mobility, or Mentation across settings of care.

What You Can Do

  • Talk to your medical provider about any side effects that you experience.
  • Ask your care team to assess how your medicines interact with natural remedies you take.
  • Tell your providers about any over-the-counter medicine or natural remedies you take.

Prevent, identify, treat, and manage dementia, depression, and delirium across settings of care.


What You Can Do

  • Tell your medical provider if you notice any changes in your memory or mood.
  • Get screened for dementia and depression each year.
  • If you become confused suddenly, get help right away.  Sudden confusion (Delirium) is a medical emergency.

Ensure that older adults move safely every day in order to maintain function and do What Matters.

What You Can Do

  • Talk to your care providers about how you get around at home or go from place to place.  Tell them about what's working and what's not.
  • Ask about how you can improve strength, flexibility, and balance.
  • Check your home for objects you can trip on or missing handrails that can cause falls.

By providing physical, emotional, and spiritual care to terminally ill persons and their families, hospice neither hastens nor postpones death. Hospice services are covered under several different insurance programs including Medicare, Veterans Administration, selected private insurance plans and some state funded programs.  Your hospice benefit covers several medications, medical supplies, and medical equipment.

Dr. Michael J. Bernardo is a family medicine doctor in Newberry, South Carolina and is affiliated with Newberry Health. He received his medical degree from Weill Cornell Medicine and has been in practice for more than 20 years. Dr. Michael J. Bernardo has expertise in treating diabetes, Alzheimer's dementia, hypertension, among other conditions.  Dr. Michael J.
